Nursery

No
admit dictations are required for babies born here on this admission, but they are
required when a newborn is transferred to SMC.

Discharge
summaries are required if babies stay in the hospital for >48 hours (this
frequently occurs if C-sec or GBS+).

For
morning report include date and time of delivery, gestational age, APGARs,
weight, & any problems/defects (meconium, + GBS, vacuum use).

A
CBC on the baby is needed on the day following delivery if vacuum assist was
required, to rule out significant bleeding secondary to birth trauma.

If
you fill out the white newborn or discharge exam sheet for the day & the
baby's doing well, then you do not have to write a progress note that day.

Get
consent yourself for circumcision, if it is wanted, even if nurses have already
done it. The attending will supervise. Remember to record for personal procedure
log. Most attendings and residents use GOMCO clamp for circ.

All
preemie boarder babies require an admission & discharge dictation. Direct
the transcriptionist to send a d/c dictation to the PCP.

Caloric
requirements for preemie boarder babies: Divide total # of cc intake in 24hrs by
30 to get # of oz. Multiply that by kcal/oz of formula. Divide this by baby's
wt. in kg. Baby should be getting at least 110-120 kcal/kg/d.
